Quick answer

Form SC2, Employee's Statement of Sickness, is the self-certification a UK employee completes to claim Statutory Sick Pay (SSP) from their employer for sickness lasting more than 4 consecutive days. It covers the first 7 days without a doctor's Fit Note; from day 8 a Fit Note is also required. You give it to your employer, not HMRC. What is Form SC2?

Form SC2 is the self-certification an employee in the United Kingdom completes to claim Statutory Sick Pay (SSP) from their employer for periods of sickness lasting more than 4 consecutive days. SC2 covers the first 7 days of absence without a Fit Note ("sick note") from a doctor. After 7 days, a Fit Note (Med 3) is required in addition. The employee submits SC2 directly to their employer, not to HMRC.

Who fills out Form SC2?

  • UK employees absent from work for more than 4 days due to sickness
  • Workers claiming SSP for the first time in a new sickness period
  • Anyone self-certifying short-term illness (up to 7 days) before a doctor's Fit Note is required

Common mistakes to avoid

  • 1Submitting SC2 to HMRC — it goes to your employer, not HMRC. HMRC publishes the form but employers process it.
  • 2Not submitting a Fit Note after 7 days — SSP can be paused if the employer doesn't receive medical evidence for longer absences.
  • 3Forgetting to include the National Insurance number — employers need it to process SSP correctly.

Frequently asked questions

SSP is set by the UK government and updated annually. Check the current weekly rate on gov.uk — it's a flat statutory amount, not based on your salary.
After 3 "waiting days" — SSP is paid from the 4th qualifying day of sickness onward, for up to 28 weeks per period of incapacity.
SC2 self-certification only covers the first 7 calendar days of sickness. From day 8 onward, you need a Fit Note (Statement of Fitness for Work) from a healthcare professional.
You can challenge the decision and ultimately escalate to HMRC's Statutory Payments Disputes Team for a formal ruling.
