Form SC2, Employee's Statement of Sickness, is HMRC's self-certification a UK employee can use to ask their employer for Statutory Sick Pay (SSP). There is no SC2 PDF to download: HMRC provides it only as a free online form on gov.uk, which you complete, print and give to your employer, not to HMRC. Since 6 April 2026 you qualify after being off sick for at least one full working day, and SSP is paid from that first day with no waiting days. The SC2 covers an absence of up to 7 days in a row; from day 8 you also need a fit note from a healthcare professional. Form SC2 is the self-certification an employee in the United Kingdom can use to ask their employer for Statutory Sick Pay (SSP). Until 5 April 2026 you had to be sick for more than 3 days in a row and sat out 3 unpaid waiting days; for absences from 6 April 2026 you qualify after one full working day and SSP runs from that day. The SC2 can be used on its own for an absence of up to 7 days in a row, including weekends and non-working days, or alongside a fit note for a longer one. After 7 days a fit note ("sick note") from a GP, hospital doctor, registered nurse, occupational therapist, pharmacist or physiotherapist is required as well. The employee gives the SC2 to their employer, not to HMRC. HMRC provides the SC2 as an online form you fill in, print and hand over; it does not publish a blank PDF. There is no official SC2 PDF to download, and there never has been on gov.uk: HMRC provides the SC2 only as a free online form, "Ask your employer for Statutory Sick Pay", reached from the gov.uk SC2 page. You sign in to the HMRC service, answer the questions (National Insurance number, phone number, clock or payroll number, the dates you were sick and your last working day before the sickness), then print or save the result and give it to your employer, keeping a copy. You cannot save part-way, so gather the details first. Sites offering an "SC2 PDF" are copies of older versions.
