No. The exported file is a normal PDF. Your client can fill it in Acrobat Reader, a browser or a phone. Only you need an account to prepare it.

Templates are tied to the exact PDF file. The same file reopens with your fields in place. A fresh scan of the same paper form is a different file, so it needs its own detection pass.
JustFill is meant for preparing blank forms and for filling forms with data you are allowed to process. Its terms do not allow health information (PHI), so do not use it for patient or medical records.
