Quick answer

HMRC Form P45, Details of Employee Leaving Work, is the four-part UK PAYE statement an employer issues when an employee stops working for them. Part 1 is reported to HMRC via Real Time Information (RTI), Part 1A is kept by the employee for their own tax records, and Parts 2 and 3 are handed to the new employer (or Jobcentre Plus) so the new employer can carry forward cumulative taxable pay, tax deducted, and tax code in the current tax year. If a new employer receives a flat P45 PDF or needs to complete the Part 3 new-starter section online — or if an employee has no P45 and needs the HMRC Starter Checklist — JustFill lets you complete and download the PDF in your browser.

What is Form P45?

Need to fill out Part 3 of a flat P45 PDF received by email from a new starter, prepare P45 leaving worksheets for payroll archiving, or figure out what every box on Parts 1A, 2, and 3 means? Under UK PAYE rules, every employee leaving a job is entitled to a P45 showing their Employer PAYE reference, National Insurance number, leaving date, tax code, student loan indicator, and cumulative pay and tax from 6 April up to their leaving date. Because emailed P45 PDFs exported from payroll software (Sage, Xero, BrightPay, Iris, QuickBooks) are almost always flat, uneditable PDFs, HR teams and new employers use JustFill to type directly into Part 3 (New Employer details) without printing and re-scanning.

HMRC requires employers with payroll software to generate P45s through Real Time Information (RTI) software (or HMRC Basic PAYE Tools). When a new starter emails you a flat P45 PDF and you need to fill in Part 3 digitally, or when you need an interactive P45 data-collection worksheet for payroll setup, click "Fill the blank Form P45 now" above or upload your P45 PDF in JustFill; your first clean completed download is included at no charge.

Who fills out Form P45?

  • UK employers and payroll administrators completing Part 3 of a new employee's flat P45 PDF for digital onboarding records
  • Accountants, bookkeepers, and payroll bureaus preparing P45 leaving worksheets before submitting final Full Payment Submissions (FPS)
  • Employees reviewing Parts 1A, 2, and 3 of their P45 to check their leaving tax code, Week 1/Month 1 status, and year-to-date pay and tax
  • New starters who do not yet have their P45 and need to know whether to complete the HMRC Starter Checklist alongside or instead of Form P45

Field-by-field breakdown

What each section of Form P45 asks for. JustFill’s AI will detect these fields automatically when you upload the PDF — review the breakdown below so you know what to enter.

Boxes 1–4 — Employer PAYE reference, Employee NI number, Name & Leaving Date

3-digit HMRC office number and employer PAYE reference, employee's National Insurance number, title, surname, first name(s), and Leaving Date in DD MM YYYY format.

Boxes 5–6 — Student Loan Deductions & Tax Code at Leaving

Checkbox indicating whether student loan deductions were continuing (`Y`), the employee's tax code at leaving (e.g. `1257L`, `BR`, `0T`), and whether it was operated on a Week 1 / Month 1 (non-cumulative) basis.

Boxes 7–8 — Total Pay to Date & Total Tax to Date (or Pay/Tax in This Employment)

Cumulative taxable pay and income tax deducted in the current tax year (left blank if the tax code was Week 1 / Month 1, in which case pay and tax in this employment only are entered in the boxes below).

Boxes 9–13 — Works/Payroll number, Gender, DOB & Employer Address/Declaration

Employee payroll number, sex, date of birth, employee private address on Part 1/1A, and the leaving employer's name, address, postcode, and certification date.

Part 3 (Boxes 14–19) — For Completion by New Employer

New employer's PAYE reference, date new employment started, new works/payroll number, starter declaration/tax code applied, and whether P45 figures are included in the new employer's first payroll run.

Common mistakes to avoid

  • 1Using a P45 from an old tax year — a P45 is only valid for the current tax year (or at the very start of a new tax year under HMRC transition rules); otherwise the new starter must complete an HMRC Starter Checklist.
  • 2Forgetting the student loan plan type — Box 5 on Form P45 only has a Yes/No indicator for student loans and does NOT state whether the employee is on Plan 1, Plan 2, Plan 4, Plan 5, or a Postgraduate Loan, so HMRC instructs employers to ask new starters with a loan to complete the student loan section of the Starter Checklist as well.
  • 3Entering figures in Boxes 7 and 8 when the leaving tax code is on a Week 1 or Month 1 basis — HMRC rules state Boxes 7 and 8 must be left blank for Week 1/Month 1 codes and only "this employment" figures entered.
  • 4Issuing a duplicate P45 after the original has been produced — employers cannot issue a replacement P45 once generated; an employee who loses their P45 fills out the HMRC Starter Checklist at their new job instead.

Form P45 example — what a filled-out form looks like

A fictional UK employee, Oliver James Example, leaves Sample Digital Ltd on 30 September 2026 during the 2026/27 tax year on cumulative tax code 1257L and hands Parts 2 and 3 of his P45 to his new employer on 5 October 2026. All values are fictional.

Employer PAYE Reference & Employee NI Number

Office No: 123 / Ref: AZ45678 · NI Number: QQ 12 34 56 C

Identifies the leaving employer's PAYE scheme and the employee's National Insurance record.

Employee Name, Leaving Date & Tax Code at Leaving

MR EXAMPLE, OLIVER JAMES · Leaving Date: 30 09 2026 · Tax Code: 1257L (Cumulative) · Student Loan: Y

Shows the cumulative personal allowance code and flags that student loan deductions were active (the new employer still checks the Starter Checklist for the specific Plan 1/2/4/5 type).

Total Pay to Date & Total Tax to Date (2026/27)

Total pay to date: £21,000.00 · Total tax to date: £2,943.00

Because the tax code is cumulative (not Week 1/Month 1), total year-to-date taxable pay and tax are carried into the new job.

Part 3 — New Employer Completion

New Employment Start Date: 05 10 2026 · New Payroll No: NW-209 · New Employer PAYE Ref: 456/BX98765

Completed on Part 3 by the new employer when onboarding the employee into payroll.

These values are made up. They are here to show what belongs on each line, not to be copied — every name, number, and address on your own Form P45 has to be your own.

Frequently asked questions

Part 1 goes to HMRC (now transmitted electronically via the employer's RTI Full Payment Submission). Part 1A is given to the employee to keep for their personal tax and Self Assessment records. Parts 2 and 3 are given to the employee to hand over to their new employer (or Jobcentre Plus if claiming benefits).
Ask your new employer for an HMRC Starter Checklist (or open `/forms/hmrc-starter-checklist` right here in JustFill). Completing the Starter Checklist lets your new employer assign the right PAYE tax code (Statement A, B, or C) instead of emergency code 0T.
Box 5 on Form P45 only tells your new employer to continue student loan deductions; it does not state whether you repay under Plan 1, Plan 2, Plan 4, Plan 5, or a Postgraduate Loan. HMRC requires new starters with a student loan to complete the student loan questions on the Starter Checklist even when handing in a P45.
Yes. Upload the flat P45 PDF emailed by your new starter into JustFill (or open our interactive P45 worksheet), type the Part 3 new-employer fields on screen, and download a clean PDF for your digital payroll records.
