What is Form 990-EZ?

Form 990-EZ is the 4-page "Short Form" annual information return designed by the IRS for mid-sized tax-exempt organizations under IRC Section 501(c), 527, or 4947(a)(1) whose gross receipts for the tax year are less than $200,000 and whose total assets at the end of the tax year are less than $500,000 (organizations normally at or below $50,000 in gross receipts can file the electronic Form 990-N e-Postcard, while organizations at or above $200,000 in gross receipts or $500,000 in assets must file the full 12-page Form 990). Because Form 990-EZ is open to public inspection under IRC Section 6104 and scrutinized by donors, grantmakers, charity watchdogs, and state attorneys general, nonprofit boards and treasurers routinely prepare and review the complete 4-page PDF — covering Part I (Revenue, Expenses, and Changes in Net Assets), Part II (Balance Sheets), Part III (Statement of Program Service Accomplishments), Part IV (List of Officers, Directors, Trustees, and Key Employees), Part V (Other Information), and Part VI (Section 501(c)(3) Only disclosures) — before transmitting or filing with state charity regulators. Common mistakes to avoid

  • 1Filing Form 990-EZ when gross receipts (Item L) are $200,000 or more OR end-of-year total assets (Part II, Line 25, Column B) are $500,000 or more — crossing either threshold requires the full Form 990.
  • 2Leaving Part I, Line 21 out of balance with Part II, Line 27, Column B — ending net assets on the revenue/expense statement must match ending net assets on the balance sheet.
  • 3Forgetting to attach Schedule A (Public Charity Status and Public Support) and Schedule O (Supplemental Information) for a Section 501(c)(3) organization.
  • 4Missing the filing deadline for three consecutive years, which triggers automatic revocation of tax-exempt status by operation of law under IRC Section 6033(j).

Frequently asked questions

A tax-exempt organization can file the 4-page Form 990-EZ if its gross receipts during the tax year are less than $200,000 AND its total assets at the end of the tax year are less than $500,000. If gross receipts are normally $50,000 or less, the organization may file the Form 990-N (e-Postcard), though many nonprofits voluntarily prepare Form 990-EZ to satisfy grantmakers and state charity regulators.
Form 990-EZ is due on the 15th day of the 5th month after the end of the organization's fiscal year. For a calendar-year nonprofit (year ending December 31), the due date is May 15. Filing Form 8868 on or before the due date grants an automatic 6-month extension (to November 15 for calendar-year filers).
Every 501(c)(3) public charity filing Form 990-EZ must attach Schedule A (Public Charity Status and Public Support). Organizations that received $5,000 or more from any single contributor generally attach Schedule B (Schedule of Contributors), and any organization explaining line items such as "Other revenue" (Line 8) or "Other expenses" (Line 16) attaches Schedule O.
For most tax-exempt organizations (other than private foundations and Section 527 political organizations), the IRS does not disclose the names and addresses of contributors listed on Schedule B, and the organization should redact contributor names and addresses from the public inspection copy of its Form 990-EZ.
