Quick answer

Standard Form 86 (SF-86), Questionnaire for National Security Positions (Rev. November 2016, OMB No. 3206-0005), is the 127-page federal background investigation questionnaire completed by military personnel, federal civilian applicants, and defense contractor employees applying for or renewing a Confidential, Secret, Top Secret, or SCI security clearance (Tier 3 and Tier 5 investigations). Although official submissions are transmitted electronically through the DCSA National Background Investigation Services (NBIS) e-App portal, applicants and Facility Security Officers (FSOs) use the SF-86 PDF as an archival pre-application worksheet to gather 10 years of residences, employment, education, foreign contacts, and financial disclosures before the e-App session expires. Open an interactive SF-86 preparation worksheet or upload your SF-86 PDF in JustFill to complete it in your browser without the XFA "Please wait..." error.

What is SF-86?

Preparing for a Secret, Top Secret, or SCI security clearance investigation as a defense contractor, military service member, or federal employee? Standard Form 86 (SF-86) is one of the most detailed questionnaires in the U.S. government, spanning 30 sections across 127 pages. Because official government SF-86 PDFs are complex Adobe forms that frequently trigger the "Please wait... If this message is not eventually replaced by the proper contents of the document" XFA screen or freeze browser PDF viewers, and because the online DCSA NBIS e-App portal times out if you do not have every 10-year street address, zip code, supervisor phone number, and neighbor verifier ready in advance, Facility Security Officers (FSOs) and clearance candidates use JustFill to complete a clean SF-86 worksheet in the browser and keep a permanent personal copy for their next 5-year Continuous Vetting (CV) / periodic reinvestigation.

Defense Contractor & FSO Workflow: SF-86 Pre-Application Worksheet & DD Form 2875 (SAAR)

For Facility Security Officers (FSOs), defense contractors, and federal/military applicants, completing Standard Form 86 (Questionnaire for National Security Positions) is the critical gate to Secret (Tier 3) and Top Secret / SCI (Tier 5) clearances. Preparing an offline SF-86 worksheet in JustFill before logging into the DCSA NBIS e-App portal prevents session timeouts and catches 30-day residence or employment gaps early.

Once your FSO submits your SF-86 in NBIS and verifies your interim or final eligibility in DISS, pair your clearance onboarding with DD Form 2875 (System Authorization Access Request / SAAR) to provision NIPRNet and SIPRNet accounts.

  • Eliminate the "Please wait... If this message is not eventually replaced" XFA viewer error on Mac, Windows, and Linux browsers.
  • Retain your completed SF-86 worksheet PDF in your personal records so your next 5-year Continuous Vetting (CV) or periodic reinvestigation starts with verified dates and addresses.

The official 127-page Standard Form 86 (Rev. November 2016) is published by OPM and DCSA at opm.gov/forms/standard-forms/. Because the 127-page government PDF often freezes browser viewers or triggers the XFA "Please wait..." error, click "Fill the blank SF-86 now" above to open an interactive SF-86 pre-application worksheet in JustFill, or upload your FSO's SF-86 PDF to organize your 10-year residence, employment, and verifier records in the browser.

Who fills out SF-86?

  • Defense contractor employees, engineers, and consultants sponsored by a Facility Security Officer (FSO) in DISS / NBIS for a Confidential, Secret (Tier 3), or Top Secret / SCI (Tier 5) security clearance
  • U.S. military recruits, officers, and enlisted personnel entering national security or sensitive occupational specialties
  • Federal civilian employees and applicants in national security sensitive positions across DoD, DHS, State Department, DOJ, DOE, and intelligence agencies
  • Cleared personnel completing Continuous Vetting (CV) updates or periodic reinvestigation renewals who need a reusable master worksheet of their 10-year history

Field-by-field breakdown

What each section of SF-86 asks for. JustFill’s AI will detect these fields automatically when you upload the PDF — review the breakdown below so you know what to enter.

Sections 1–10 — Full Name, DOB, Place of Birth, SSN, Other Names, Contact Info, Passport & Citizenship

Legal name, date and place of birth, SSN, all prior names or aliases with dates used, email/phone numbers, U.S. passport number and issue/expiration dates, U.S. citizenship status (by birth, naturalization, or derivative), and dual/foreign citizenship or foreign passport history.

Section 11 — Where You Have Lived (10-Year Residence History)

Every physical street address (no P.O. boxes alone; use APO/FPO plus physical base location for overseas military) for the last 10 years without a single 30-day gap, whether owned/rented/military, and a neighbor or landlord verifier (name, phone, email, and current street address) for every residence within the last 3 years.

Section 12 — Where You Went to School & Section 13A — Employment Activities (10-Year History)

All schools attended in the last 10 years plus any degrees/diplomas earned over 10 years ago (with a verifier for schools in the last 3 years), and every employment, self-employment, active-duty military, federal contractor, or unemployment period over the last 10 years with no gaps, including supervisor name, title, email, phone, and physical work address.

Sections 14–18 — Selective Service, Military History, 3 People Who Know You Well, Marital Status & Relatives

Selective Service registration number, U.S. or foreign military service and discharge type, three non-relative social references who collectively cover the last 7 years, current/former spouses or cohabitants, and full biographic/citizenship details for mother, father, stepparents, foster parents, children, brothers, sisters, and in-laws.

Sections 19–20C — Foreign Contacts, Foreign Financial Interests, Foreign Business & Foreign Travel

Close and/or continuing contact within the last 7 years with any foreign national with whom you or your spouse are bound by affection, influence, common interests, or obligation; foreign bank accounts, real estate, or business ties; and every non-official foreign travel trip in the last 7 years.

Sections 21–30 — Psychological Health, Police Record, Drug/Alcohol Use, Investigations, Finances, IT Misuse, Civil Court, Associations & Releases

Reportable criminal history (ever-scope for felonies, firearms/explosives, domestic violence, or alcohol/drug offenses; 7-year scope for other offenses), illegal drug use/involvement in the last 7 years (including federal law rules on marijuana/THC regardless of state law), financial delinquencies/bankruptcies/tax liens in the last 7 years, prior clearance history, and the four Authorization/Fair Credit Reporting/Medical Release forms in Section 30.

Common mistakes to avoid

  • 1Leaving chronological gaps of 30+ days in Section 11 (Residences) or Section 13A (Employment) — every single month of the past 10 years must be accounted for (if unemployed between jobs or in school, explicitly add an Unemployment entry with a verifier who can confirm what you were doing).
  • 2Listing the same person as a Residence Verifier (Section 11), Education Verifier (Section 12), and "Person Who Knows You Well" (Section 16) — DCSA investigators strongly prefer distinct, non-relative verifiers for each activity.
  • 3Assuming state-legal cannabis/marijuana or CBD/THC products do not need to be disclosed in Section 23 — security clearances are governed by federal law (Controlled Substances Act), so omitting recent THC use is treated as falsification/lack of candor under Adjudicative Guideline E (Personal Conduct), which is far more disqualifying than mitigated prior use.
  • 4Entering a corporate headquarters address in Section 13A instead of the physical street address where you actually reported to work (and listing the staffing company without also listing the physical client/government site).
  • 5Failing to save a personal offline copy of your completed SF-86 worksheet — when your clearance comes up for a 5-year Continuous Vetting update or Tier 5 upgrade, having your exact prior dates and verifier addresses prevents accidental discrepancies.

SF-86 example — what a filled-out form looks like

A fictional defense contractor aerospace engineer prepares her SF-86 pre-application worksheet before her Facility Security Officer (FSO) initiates her Tier 5 (Top Secret) investigation in DCSA NBIS e-App. Every name, address, and identifier is fictional.

Sections 1–9 — Personal Identity, Citizenship & U.S. Passport

SAMPLE, RACHEL A · DOB: 03/22/1992 in Denver, CO · Citizenship: U.S. Citizen by birth · U.S. Passport Issued: 05/14/2021, Expires: 05/13/2031 · Dual Citizenship: No

Establishes identity and U.S. citizenship before entering the 10-year chronological history.

Section 11 — 10-Year Residence History (Current Residence)

2023-06 to Present · 1420 Sample Liberty Way Apt 302, Arlington, VA 22202 · Rented · Neighbor Verifier: Thomas Sample, 1420 Sample Liberty Way Apt 304, Arlington, VA 22202, (703) 555-0191

Every residence within the past 3 years requires a neighbor, landlord, or acquaintance verifier with a complete physical street address and phone number.

Section 13A — 10-Year Employment Activities (Current Defense Contractor Role)

2022-08 to Present · Federal Contractor · Employer: Sample Aerospace Systems Corp · Physical Work Location: 2100 Sample Pentagon Blvd, Arlington, VA 22202 · Supervisor: Elena Sample, Engineering Director, (703) 555-0144

Lists both the employing defense contractor and the physical work location, plus a direct supervisor verifier.

Section 16 — Three People Who Know You Well

Reference 1 (2018–Present): Kevin Sample · Reference 2 (2019–Present): Priya Sample · Reference 3 (2020–Present): Daniel Sample — non-relatives not already listed as Residence/Employment verifiers

Section 16 requires three non-relative social contacts who collectively cover the past 7 years.

Section 20B — Foreign Travel & Section 25 — Prior Investigations

Foreign Travel: 2024-07 (8 days, Tourism in Italy & France, personal funds) · Prior Investigation: Tier 3 Secret clearance granted by DoD CAF / DCSA CAS on 2021-11-04

Discloses all non-official foreign travel over the past 7 years and prior clearance adjudication history.

These values are made up. They are here to show what belongs on each line, not to be copied — every name, number, and address on your own SF-86 has to be your own.

Frequently asked questions

DCSA's online NBIS e-App (which replaced e-QIP) has strict session timeouts and requires dozens of exact 10-year street addresses, ZIP codes, supervisor phone numbers, relative birthdates, and neighbor verifier addresses. Facility Security Officers (FSOs) routinely have candidates complete the SF-86 PDF worksheet first so the FSO can review it for 30-day timeline gaps before initiating the live e-App request, and so the candidate retains an offline reference copy for future Continuous Vetting renewals.
Some government mirrors distribute Standard Form 86 or its signature addenda as Adobe LiveCycle XFA PDFs that cannot render inside Chrome, Edge, Firefox, Safari, or macOS Preview. You can open an interactive SF-86 preparation worksheet directly in JustFill using the button above, or use our XFA PDF to Fillable PDF converter (/xfa-pdf-to-fillable-pdf).
On the current Rev. November 2016 edition of SF-86, Sections 11 (Where You Have Lived), 12 (Where You Went to School), and 13A (Employment Activities) require a continuous 10-year history (though you do not need to list occurrences before your 18th birthday as long as you provide at least 2 full years of history). Most security/suitability sections — such as Section 16 (People Who Know You Well), Section 19 (Foreign Contacts), Section 20B (Foreign Travel), Section 23 (Drug Use), and Section 26 (Financial Record) — ask for a 7-year window alongside specific "Ever" questions.
SF-85 is used for Non-Sensitive (Tier 1 / HSPD-12 PIV credential) federal positions; SF-85P is used for Public Trust (Moderate or High Risk, Tier 2 / Tier 4) positions; and SF-86 is used for National Security Positions requiring a Confidential, Secret (Tier 3), or Top Secret / SCI (Tier 5) security clearance.
Yes. For military and DoD contractor personnel, once your FSO verifies your interim or final clearance eligibility in DISS (resulting from your SF-86), you complete DD Form 2875 (System Authorization Access Request / SAAR) so the command Security Manager and ISSO can authorize your NIPRNet or SIPRNet system accounts.
