Field-by-field breakdown

What each section of HMRC Form R40 asks for. JustFill’s AI will detect these fields automatically when you upload the PDF — review the breakdown below so you know what to enter.

Section 1 — Personal Details & Tax Year Claimed

Tax year ending 5 April for which the claim is made (1 form per tax year), your full name, UK National Insurance number (NINO), date of birth, current address and postcode, and daytime telephone number.

Sections 2 & 3 — Employment, Pensions & State Benefits (Boxes 2.1–3.10)

Total gross pay and tax deducted from all employments (per P60/P45), occupational/private pensions, State Pension (Box 3.1), and taxable state benefits (such as Bereavement Allowance or Carer’s Allowance) received during the tax year.

Sections 4–6 — UK Savings Interest, Purchased Life Annuities & Trust/Estate Income

Net interest/annuity received, tax deducted at source, and gross amount before tax (Boxes 4.1–4.5), untaxed UK bank/building society interest (Box 4.6), and UK trust, settlement, or estate income from Form R185 (Boxes 5.1–6.6).

Sections 7–10 — Foreign Income, Chargeable Events, Gift Aid & Blind Person’s Allowance

Foreign dividends or interest, chargeable event gains on UK life insurance policies, Gift Aid charitable payments, and Married Couple’s / Blind Person’s Allowance claims.

Sections 11 & 12 — Repayment Instructions (Bank Transfer / Nominee) & Declaration

UK bank or building society account holder name, 6-digit sort code, 8-digit account number (or nominee details in Box 11.4), and claimant signature and date in Box 12.2.

Common mistakes to avoid

  • 1Combining multiple tax years onto a single Form R40 — HMRC requires a separate Form R40 for each tax year (from 6 April to 5 April) you are claiming.
  • 2Omitting your State Pension in Box 3.1 — even though the Department for Work and Pensions (DWP) pays the State Pension without deducting PAYE tax, it is taxable income that uses up part of your £12,570 Personal Allowance.
  • 3Leaving Box 4.6 (untaxed UK interest) blank when you also earned interest on ordinary bank or building society accounts paid gross.

HMRC Form R40 example — what a filled-out form looks like

A fictional retired UK taxpayer in Bath completes HMRC Form R40 for the 2025–2026 tax year to reclaim £640 of Income Tax deducted at 20% from a purchased life annuity and a family trust distribution.

Section 1 — Personal Details & Tax Year

Tax Year Ended: 05/04/2026 · Name: Mrs Margaret H. Sample · NINO: QQ 78 90 12 B · DOB: 22/03/1952 · Address: 9 Circus Mews, Bath, BA1 2EW

Fictional NINO; each tax year requires its own Form R40.

Sections 2 & 3 — Pensions & State Pension

Box 2.3 (Private Pension Gross): £4,200.00 (Tax off: £0.00) · Box 3.1 (State Pension received in year): £7,800.00

Sections 4 & 5 — Taxed Annuity & Trust Income

Box 4.1 Net: £2,560.00 · Box 4.2 Tax deducted: £640.00 · Box 4.3 Gross: £3,200.00 · Box 4.6 Untaxed bank interest: £310.00

Total income (£15,510) falls within the £12,570 Personal Allowance plus £5,000 Starting Rate for Savings, entitling her to a full £640 repayment.

Sections 11 & 12 — Bank Details & Signature

Account Name: M H Sample · Sort Code: 40-XX-XX · Account No: 1234XXXX · Signed: Margaret H. Sample · Date: 23/10/2026

These values are made up. They are here to show what belongs on each line, not to be copied — every name, number, and address on your own HMRC Form R40 has to be your own. Keep your annual tax deduction certificates (such as R185 or annuity statements) on file in case HMRC requests verification.
