What is TX Form VTR-34?

Lost your paper Texas vehicle title before trading in or selling your car or truck? Form VTR-34 is the official Texas Department of Motor Vehicles application for a Certified Copy of Title (CCO). If the title shows a recorded lien, the lienholder must either apply for the certified copy or provide an original Release of Lien (Form VTR-266) on letterhead with an acceptable photo ID. JustFill lets Texas vehicle owners, auto dealerships, and fleet title clerks complete Form VTR-34 cleanly in the browser and pair it with Form VTR-271 (Power of Attorney) or Form 130-U.

Field-by-field breakdown

What each section of TX Form VTR-34 asks for. JustFill’s AI will detect these fields automatically when you upload the PDF — review the breakdown below so you know what to enter.

Submission Method & Reason for Replacement

Selection of Mail-In ($2.00 fee) or In-Person Regional Service Center ($5.45 fee), plus reason checkboxes (Lost, Stolen, Destroyed, Damaged/Surrendered, or Never Received).

Vehicle Identification

17-character Vehicle Identification Number (VIN), year, make, body style, Texas license plate number, and Texas Title/Document number (if known).

Owner / Lienholder & Mailing Address

Recorded owner(s) or lienholder name, mailing address where the Certified Copy of Title should be sent, and recipient ID details.

Signatures & Government Photo ID Information

Original signature(s) of all recorded owners (or authorised agent/lienholder), photo ID type, ID number, issuing state, and date.

Common mistakes to avoid

  • 1Forgetting to enclose a clear photocopy of the government-issued photo ID for every owner or agent who signs Form VTR-34 — TxDMV rejects VTR-34 applications without photo ID copies.
  • 2Having only one spouse or co-owner sign when the Texas title record lists two owners without "OR" / rights of survivorship.
  • 3Mailing Form VTR-34 to a local County Tax Assessor-Collector office instead of a TxDMV Regional Service Center (County tax offices process 130-U transfers, whereas Certified Copies of Title are issued only by TxDMV).

TX Form VTR-34 example — what a filled-out form looks like

A fictional Austin truck owner applies in person at the Austin TxDMV Regional Service Center for a Certified Copy of Title after misplacing the original paper title following loan payoff. All names and numbers are fictional.

Vehicle & Replacement Reason

Reason: Lost · Submission: In Person ($5.45) · VIN: 1FTFW1E58MFA10293 · 2021 Ford F-150 PK · Plate: RKP-8821

Identifies the Texas title record to be reissued as a Certified Copy.

Recorded Owner & Mailing Address

Owner: SAMPLE, TRAVIS COLE · Address: 4102 Sample Mesa Dr, Austin, TX 78731 · Lien Released: Yes (VTR-266 attached)

Confirms the recorded owner and provides the original lien release so the CCO issues clear of liens.

Applicant Photo ID & Signature

ID Type: TX Driver License #19482710 · Exp: 08/15/2029 · Signed by Owner: Travis C. Sample, 10/21/2026

Satisfies TxDMV’s mandatory photo ID verification for Certified Copy of Title issuance.

These values are made up. They are here to show what belongs on each line, not to be copied — every name, number, and address on your own TX Form VTR-34 has to be your own.

Frequently asked questions

Unlike Form 130-U (which goes to your county tax office), Form VTR-34 must be submitted either by mail to the Texas Department of Motor Vehicles (1601 Southwest Parkway, Suite A, Wichita Falls, TX 76302) with a $2.00 fee, or in person at a TxDMV Regional Service Center with a $5.45 fee.
If TxDMV records show an active lien, only the recorded lienholder can obtain a Certified Copy of Title unless you attach an original, signed Release of Lien (Form VTR-266 or lender letterhead) along with a photocopy of the lender representative’s photo ID and employee ID/business card.
Under Texas Transportation Code rules, TxDMV cannot issue more than one Certified Copy of Title within a 30-day period.
