How to fill multiple PDF forms from Excel

Keep the original PDF layout. Put column names in the first spreadsheet row and one customer, employee, vendor, or case in each row below it. JustFill creates a separate PDF for every selected row.

  1. 1

    Upload the original PDF

Use an AcroForm, a flat PDF, or a scan. JustFill detects the fields before the merge.

  2. 2

    Import XLSX or CSV

Dates, leading zeros, percentages, booleans, and spreadsheet headers are preserved for mapping.

  3. 3

    Check the column mapping

Matching headers map directly. Ambiguous columns can use AI-assisted mapping.

  4. 4

    Review and export

Approve the records you want and download individual filled PDFs in one ZIP.

Where it fits

Built for outpatient clinics, dental practices, mobile health events, and care coordinators.

When patients arrive for a scheduled clinic day, employer occupational-health screening, or nursing-home mobile dental/vision visit, pre-filling known demographics, insurance subscriber IDs, and provider details onto your practice’s existing PDF intake packet eliminates clipboard bottlenecks and illegible handwriting.

Paid workflow

Turn flat paper clinic forms into reusable digital & batch templates.

Save your practice’s reviewed intake and consent layouts once. Use Batch Fill for daily schedule rosters or Shareable Fillable Links so patients can fill and e-sign on their phone before arriving.

Works on scanned & Word-exported clinic forms

Most clinic and dental forms are flat PDFs or paper scans. D-FINE visual detection finds every text line, medical-history checkbox, and signature area.

Mobile-friendly Patient Fill & E-Sign Links

Turn any saved clinic PDF into a public justfill.app/f/<token> link so patients can complete medical history checkboxes and e-sign from any smartphone.

Multi-PDF New Patient Packets

Bundle Registration + Medical History + HIPAA Consent + Financial Policy into one Multi-PDF Packet so patient demographics populate across every page.

Clear boundaries

JustFill is a general-purpose PDF form-filling utility and is not an EHR, medical device, healthcare clearinghouse, or HIPAA Business Associate by default. Always use synthetic sample data during initial template setup, review our Data Processing Agreement (/dpa), and process only records your organization is authorized to handle.

Frequently asked questions

How do clinics batch pre-fill patient intake PDFs from a daily appointment schedule?

Upload your clinic’s PDF intake or consent form to JustFill, calibrate the detected fields once using synthetic sample data, and import your daily appointment CSV or Excel sheet. JustFill populates one PDF per scheduled row and exports them in a clean ZIP ready for check-in.

Can JustFill detect checkboxes on flat or scanned medical history forms?

Yes. Medical and dental intake forms frequently contain dozens of symptom, allergy, and surgical-history checkboxes. JustFill’s computer-vision detector locates both text lines and checkboxes on flat or scanned PDFs so you can export an interactive AcroForm or fill them in batch.

Can patients fill out and e-sign our existing clinic PDF on their phone before their appointment?

Yes. Once you save your clinic intake PDF as a template in JustFill, you can generate a shareable link (justfill.app/f/<token>) where a patient fills out text boxes, checks medical history boxes, and draws their e-signature in their mobile browser, producing a completed PDF with an audit stamp.

What if we also need to batch-fill CMS-1500 or UB-04 billing claim PDFs?

JustFill has a dedicated workflow for revenue cycle and billing teams — see our CMS-1500 & UB-04 Medical Claim PDF Automation page (/solutions/cms-1500-ub-04-medical-claim-pdf-automation) for Boxes 1–33 and Form Locators 1–81.

Should we use synthetic data when setting up our clinic template?

Yes. Always calibrate your PDF template and test column mappings using synthetic sample rows (like the downloadable CSV template on this page) before running authorized operational batches.

Does it work with scanned or flat PDFs?

Yes. JustFill detects field locations in fillable, flat, and scanned PDFs before mapping spreadsheet columns.

Does each spreadsheet row create a separate PDF?

Yes. The first row contains the Excel or CSV column names. Every selected data row becomes one reviewable PDF in the exported ZIP.

Is this the same as Word mail merge?

It uses the same one-row-per-document idea, but fills your existing PDF directly. You do not need to recreate the form in Word or convert the PDF first.

Can I review results before downloading?

Yes. Each spreadsheet row becomes a reviewable record. Approve the records you want before creating the ZIP.

Does JustFill email the generated PDFs?

Not in the current self-service batch. It creates individual PDFs and packages the approved files in a ZIP for download.

What happens to uploaded documents?

Transfers use TLS. Rendered page images are deleted within 7 days and source documents within 30 days unless you save the PDF as a reusable template.
