PDF mail merge, without Word

How to fill multiple PDF forms from Excel

Keep the original PDF layout. Put column names in the first spreadsheet row and one customer, employee, vendor, or case in each row below it. JustFill creates a separate PDF for every selected row.

  1. 1

    Upload the original PDF

Use an AcroForm, a flat PDF, or a scan. JustFill detects the fields before the merge.

  2. 2

    Import XLSX or CSV

Dates, leading zeros, percentages, booleans, and spreadsheet headers are preserved for mapping.

  3. 3

    Check the column mapping

Matching headers map directly. Ambiguous columns can use AI-assisted mapping.

  4. 4

    Review and export

Approve the records you want and download individual filled PDFs in one ZIP.

Upload your CMS-1500 (HCFA) or UB-04 PDF template

Start with a standard CMS 1500 fillable PDF (Boxes 1–33) or a UB 04 form PDF (CMS-1450 Form Locators 1–81), whether interactive AcroForm or flat.

Import your billing export from Excel or CSV

Run an HCFA 1500 batch fill from CSV or XLSX by mapping patient demographics, insured ID, ICD-10 diagnosis pointers (Box 21), CPT/HCPCS service lines (Box 24A–J), tax ID, and rendering/billing NPI (Boxes 25–33).

Inspect every claim on the canvas and export a ZIP

Verify box alignment, modifiers, charges, and NPI numbers on each generated claim draft before downloading a clean ZIP for printing or payer portal upload.

Where it fits

Built for medical billing claim PDF automation — HCFA 1500 batch fill from CSV and UB-04 institutional runs.

When out-of-network payers, workers’ compensation carriers, auto/PIP insurers, secondary payers, or paper appeal packets require a clean CMS 1500 fillable PDF (02/12 edition, Boxes 1–33) or UB 04 form PDF (CMS-1450, Form Locators 1–81), JustFill turns your practice management or EHR spreadsheet export into individual, print-ready claim PDFs in one batch.

CMS 1500 fillable PDF (02/12 professional claim) batch generation
UB 04 form PDF (CMS-1450 institutional claim) outpatient & facility runs
HCFA 1500 batch fill from CSV for out-of-network, workers’ comp & auto/PIP claims
Secondary claim, corrected claim (Frequency Code 7), and appeal packet batches
DME, ambulance, behavioral health, and therapy recurring claim schedules
CMS-1490S patient-submitted Medicare claim form preparation

Clear boundaries

JustFill is a PDF drafting utility. It is not a healthcare clearinghouse, medical coding service, or HIPAA covered entity, does not validate CPT/HCPCS or ICD-10-CM coding accuracy, does not determine medical necessity, and does not transmit EDI 837P/837I claims to payers. Use synthetic data during initial template setup and process only data your organization is authorized to handle.

Frequently asked questions

How does HCFA 1500 batch fill from CSV work with a CMS 1500 fillable PDF?

Upload your CMS 1500 fillable PDF (or flat CMS-1500 template) to JustFill, import your CSV or Excel file where each row represents one claim, and map your columns to Boxes 1–33 (patient info, subscriber ID, ICD-10 codes in Box 21, CPT/HCPCS lines in Box 24, and provider NPI/TIN in Boxes 25–33). Preview each claim in the browser and export all populated PDFs in one ZIP.

Does JustFill also support the UB 04 form PDF (CMS-1450) for institutional claims?

Yes. You can upload a UB 04 form PDF (CMS-1450), auto-detect or calibrate Form Locators 1 through 81 (including revenue codes, HCPCS rates, value codes, and attending/operating provider NPIs), save the layout as a reusable template, and batch-populate institutional claims from Excel or CSV.

Can we keep our Billing Provider Name, Address, EIN, and NPI (Boxes 25, 32, and 33) constant across every claim?

Yes. Include your standard facility and billing provider columns in your CSV export or pre-fill them on your saved template so every generated CMS-1500 or UB-04 PDF automatically carries your practice’s EIN, NPI, and taxonomy code.

Does JustFill submit claims electronically to Medicare or commercial clearinghouses?

No. JustFill is a browser-based medical billing claim PDF automation tool that generates populated PDF files in a ZIP for paper mailing, faxing, payer portal upload, or appeal packets.

Does it work with scanned or flat PDFs?

Yes. JustFill detects field locations in fillable, flat, and scanned PDFs before mapping spreadsheet columns.

Does each spreadsheet row create a separate PDF?

Yes. The first row contains the Excel or CSV column names. Every selected data row becomes one reviewable PDF in the exported ZIP.

Is this the same as Word mail merge?

It uses the same one-row-per-document idea, but fills your existing PDF directly. You do not need to recreate the form in Word or convert the PDF first.

Can I review results before downloading?

Yes. Each spreadsheet row becomes a reviewable record. Approve the records you want before creating the ZIP.

Does JustFill email the generated PDFs?

Not in the current self-service batch. It creates individual PDFs and packages the approved files in a ZIP for download.

What happens to uploaded documents?

Transfers use TLS. Rendered page images are deleted within 7 days and source documents within 30 days unless you save the PDF as a reusable template.
