Form MC-030 is the Declaration approved for optional use by the Judicial Council of California. It is the general-purpose form for making a written statement to a court that you swear under penalty of perjury is true. Unlike MC-031 it stands on its own rather than being attached to another paper. You identify the court, the parties and the case number, state your role, write the declaration, then date and sign it.
