Form FW-001 is the Request to Waive Court Fees, a confidential mandatory Judicial Council form. The court may order you to answer questions about your finances, and a waiver is not always permanent — you may have to pay later if you cannot prove eligibility, if your finances improve, or if you settle a civil case for $10,000 or more.
