Form DS-11 is the U.S. Department of State application used to apply for a U.S. passport book and/or card in person at an acceptance facility, passport agency, or U.S. embassy or consulate. You must use DS-11 (rather than the DS-82 renewal form) if you are applying for the first time, you are under age 16, your most recent passport was issued when you were under 16 or more than 15 years ago, or your prior passport was lost, stolen, or damaged. The form collects your name, date of birth, sex, place of birth, Social Security number, contact details, mailing address, prior names used, parental information, emergency contact, and travel plans, and ends with an oath and signature block.
